You don't want to look like a big dork walking in with a big old GIFT BOX, (especially if no one else is doing the presents thing), so just bring her a nice Birthday Card with like a "Starbucks" gift card inside! A friend's "date" did that for me at my Surprise Birthday party, I didn't know him, but I thought it was a VERY NICE gesture! REMEMBER ALSO: a nice bottle of champagne works too, for almost ANY occasion!! =)

If you've just started dating and you don't know her sister. I would just go with your g/f,maybe buy her sis a b/d drink!
