Just say...
"Excuse me _____, do you have some free time to talk?"
Then just let it all out.... that's their job... they're happy to help.

I was never very good at this part. I would usualy make an apointment and then not get to the point. I used the first apointment to see if this was a person that I could eventualy come to have some trust in. I have major trust issues. Then I would usually leave a detailed note explaining the real reason that I had made the appontment.
While I do not recommend this method for you, it does present a good suggestion. Many people are scared of revealing thier problems, especially for the first time. Writing before the appointment can help organize your thoughts and if you freeze while you are in the office you can always simple read from what you wrote like a script.

Good for you! Your counselor is there to talk and help you. Depending on what forms of communication you use at your school, you could email her saying you'd like to set up an appointment to talk with her. You could leave her a voice mail or you could write her a note and stick it under her door (if it is only her office).

Confront means to approach in a hostile way, btw... if you need help from your school counselor, make an appointment - it's their job to help you

Listen, the counselors are there for you. They don't get certified for being irritable jerks who don't help anyone, so just find one (heck, find your favorite one) and ask them if you two can talk some time.

Just go up to her and ask if she has a minute to talk, then tell her your problem. She HAS to listen to you and offer advice, it's her job.

It should not be difficult to talk to a counsellor. Just go and say it. Brake the verb.
