get a form from the post office - fill it out
go get your picture taken at a place that does passport photo's (about $10)
take 2 checks - one for the post office & the other that needs to be mailed (the one needs to be a real paper check, the other can be a check card, visa, whatever) (can't remember fully but it's like $110 total)
take the above and your original (or certified copy) of your birth certificate to the post office andt hey do the rest.

it takes a good 2 months but can be expediated for a fee more quickly. a friend got lucky and her's came back within 4 weeks!!

they are generally good for 10 years and the new ones are beautiful!

BTW - this is for an American citizen looking for a US passport.

Go to your post office or county courthouse to fill the application. Some travel agencies can also provide you with the forms.

It usually costs about $80 and you can pay extra to get it expedited (if you plan to travel immediately).

Bring your picture ID/driver license, original birth certificate, and two passport photo's (you can have them made at Wal-Mart for $10). You should also note your social security number and also know where your parents were born and their birthdates as well as the days and info about your spouse if married.

The passport is good for ten years.
