Absolutely not. However, "insult" is a mental construct and is also just as dependent on the condition of the mind perceiving the words as "insulting" or not. If someone's so disturbed that they perceive insult in every little thing, then no matter how much correct speech and action is used, the person will still get angry.

Anger is just as much personal responsibility as it is for one to be altruistic.

No, of course not. Some people find the very fact that anyone has an opinion insulting. There is no accounting for what anyone considers insulting. Some men are insulted if a woman voices an opinion. Some men are insulted by the very presence of women in certain situations.

No, of course not. And asking questions which are perceived as insulting will not quell anger, either.
