Make several copies and give one to your Doctor, and if you have a file at a Hospital that you regularly go to have them put a copy in your hospital file, give one to a husband/wife, and give one to your attorney if you have one, and also give one to at least 2 of your children if any and carry one in your wallet so when you need it; it will be there available at all times even if no one else is around at the time of an emergency. this does work also, and it saves on attorneys fees, I suggested this way because my medical will has did for me what I wanted and I have copies placed where I needed them to be placed so there would not be any misunderstandings of my wishes, and no mistakes about what I want. The hospital or doctor's office can do that for you. They will get the forms and then the patient has to sign them.
It's the same all over. The US, UK, Scotland, Asia, etc. Those are the 2 places to get that done. If the patient is in the hospital, then ask the nurse--she'll get it from the doctor. If at home/hospice, call the doctor's office.
