When my current boyfriend had this problem, he started asking one of my close friends how to ask me. It worked.

Some other ideas though:
Pick a date that has less pressure, like a movie, where akward conversation isnt forced, and you have the movie to talk about at the end. Just something simple, like "Hey, would you like to see a movie sometime?" Another idea, is to have a friend with you when you ask, for a little confidence. My boyfriend did this as well, and his best friend jsut slowly wandered off until Paul thought he had enough courage.

Remember though, if she says she's busy, there's a good chance she really is! So, if she turns you down (because of this or other reasons), dont ask her for a different date right away, give it a week, and then ask again if there's a night when she'd be free to go do something with you.

Good luck, you seem like a nice guy, and being scared for it seems like you really like her. Hopefully she sees that.

Do you enjoy movies? If so start up a conversation about some movie you like, then ask her if she wants to join you for a movie sometime. Music is always a good conversation piece also.
Beyond that its up to you to talk to her, food sometimes makes a good conversation piece. Take her out for a bite after the movie.
Good luck

Trevor- I like that name. I have to agree with Chris, ask her if she would like to go to the movies, and don't freak out if she hesitates, she probably has to have it approved by her parents. Tell her that you were planning on going to the movies on Friday night, that way you set a day, tell her the name of the movie. Ask her if you should pick her up or if her parents will be bringing her to the movies, that lets her know that you don't have any bad intentions and her parents will like that to. Always be courteous to her parents.

You can have one of your friends do it or one of her friends if the friend knows you like her. Or if you want to do it yourself you can do it on-line because then it is not face to face.

Seriously now... don't do it until you have built up enough confidence and acquired enough wits...
