Benefits for infertility vary widely, depending on the plan you are enrolled in, and the state in which your insurance was written. In CT, there is a state mandate that requires all group health plans to provide some benefits for infertility.

Check with your state, and check with United to see if your plan includes this benefit. There is no way we can give you a definitive answer here, with the information provided.

It can depend on whether you're on a private policy, or a group policy. If the employer your insurance is through selected a package for its employees that contained a fertility benefit, then yes... If not, no... There's no real way to tell without looking through your benefit book, or calling United. (Most insurance companies offer packages with an fertility based benefit.)

Call member services on the back of your card and ask them specifically for your plan. Because there's hundreds of variations of United HC, it's impossible to answer that here.

This coverage depends on the plan you have. You would be better asking this question to UHC direct.
