If you are fortunate enough to have a seperate living room and family room, the living room is uaually reserved for formal occasions, such as when company comes by. A family room is intended to be used every day, and this is where the television would be, as well as other recreation items, such as a pool table or a dart board.

No, they are both entirely different rooms. When it come sto living rooms, you can have a formal living room, and a family room. Both are the same, but one is basically used for doing the actual living in (watching TV, etc...), and of course the other is the one with the fancy furnitue you never use. Or.... you could have just a living room, which is where you watch TV, etc...
A dining room, formal or otherwise is where you do your eating. A formal dining room is a separate room where your dining room table is at. A dining room and a living room can be combined into one room. Such is the case in my house. Although it is considered as 1 room, both areas are entirely separate. In such a case its considered a dining area and living room area. I know, its a gray area.

In living rooms people are usually resting or doing some other activity, such as reading or drawing. In the dining room, usually you are dining or having a party of some kind.

If you have one large open space, it is a good idea to divide it into zones based on use. therefore, assign an area for dining, put table and chairs there, and a separate area for living, (couch, and chairs). That way you can enjoy your meal and then leave the table, and have separation from your dishes. This makes for a cozier environment. If you have separate rooms, and can assign one for dining, usually the one closest to the kitchen is best as the food can stay warmer longer.
