1) $4500 in medical. Probably no help as it must be over 7.5% of your income and it is an itemized deduction.
2) $7000 in child care. Probably a $600 credit if it is for one child; $1200 credit if it is for two.
3) Use Car - No deduction
4) Gas - No deduction

Maybe, it is time to see a professional.

You won't be able to claim the medical bills - you can only take medical bills that are OVER 7.5% of your income, and yours aren't.

The child care bills should entitle you to a child care credit.

You can't take expenses for driving to work, so not having gas receipts doesn't matter.

If you itemize, and opt to claim sales tax rather than state and local taxes, then you could claim sales tax paid on the car in addition to the amount in the table for your state.

The car expenses/gas or mileage can only be deducted if you are self-employed or own your own business and traveled for business purposes. Regular driving to and from work does not count. If you had to buy supplies for work and was not reimbursed you might have some deductions but you will have to have enough to be greater than the standard deduction for it to matter. If you own your home there are taxes that you can try to deduct.

For medical, it may not be enough for the required 7.5% but also factor in any therapist appts which would be deductible, mileage to and from medical, mileage to and from charity work, charity receipts. Good luck.
