If you 'fell out of visa status' there is some reason your current visa is not valid, so you either have to fix that or get a new visa. Can't quite tell what your circumstance is.

Yes you do, every time your visa expires you need to do that. Now, giving the visa status again, depends largely on why you fell out of it in the first place.

If you "have"(?) to travel, then you go to your nearest INS office or port and ask for a permit or an extention. I have no idea what status you have, but many people awaiting their status cannot leave the US until their status is adjusted, so find out, because you could be deporting yourself???
