It depends on the version of the Bible that you use but it is generally once. Please find some references from the versions I found it in:
Isaiah 14:12 Darby
(12) How art thou fallen from heaven, Lucifer, son of the morning! Thou art cut down to the ground, that didst prostrate the nations!
Isaiah 14:12 KJV+
(12) How349 art thou fallen5307 from heaven,4480, 8064 O Lucifer,1966 son1121 of the morning!7837 how art thou cut down1438 to the ground,776 which didst weaken2522, 5921 the nations!1471
Revelation 2:5 MSG
(5) Do you have any idea how far you've fallen? A Lucifer fall! "Turn back! Recover your dear early love. No time to waste, for I'm well on my way to removing your light from the golden circle.
Isaiah 14:12 Webster
(12) How art thou fallen from heaven, O Lucifer, son of the morning! how art thou cut down to the ground, which didst weaken the nations!
all the best on your search!

Well, once in the KJV. It really means morning star. It was used in a taunt to a Babylonian King and to describe Jesus.
NASB:
Isaiah 14
12 "How you have fallen from heaven, O star of the morning, son of the dawn! You have been cut down to the earth, You who have weakened the nations!
KJV
Isaiah 14
12 How art thou fallen from heaven, O Lucifer, son of the morning! how art thou cut down to the ground, which didst weaken the nations!
Funny thing is Jesus calls himself a morning star in revelation. I guess the bible translators translated the KJV according to their doctrine which states that Lucifer was Satan. I don't buy that one bit anymore. Read the whole chapter of Isaiah 14 and you'll see what I am talking about.
Revelation 22:16
"I, Jesus, have sent My angel to testify to you these things for the churches. I am the root and the descendant of David, the bright morning star."
I am really not sure why people say Lucifer was Satan.

The others are correct. The reference from Isaiah, however, is not in reference to Satan. If you read the chapter, the author is very explicitly talking about the King of Babylon, a real person who was alive at the time of the writing.
