Most people mean that you look healthy. But some say it sarcastically and mean you don't look well, you've put on weight. If the person you know is implying this one then I would ditch them like a hot potato.

It can mean that, yes. The more direct way:
Tell me, when you look for a new car have you decided to look first at a Ford Galaxy, a Chelsea Tractor, or are you going to have a go at a lorry right off...

I imagine it could be, but I think that generally, it's just something people say when they haven't seen someone for a while. If they didn't think you "look well" (or that you've gained weight" they may be more inclined to just say "It's good to see you" or something like that.

A friend I hadnt seen in a long time said to me I look well, she also liked my new hair colour and said my face looked thinner, so its something someone would say to you, stating they think you are looking good and well and healthy

No-I think it means exactly what they have said. I usually take people at face value and if someone says that to me then I take it that they mean it. A good positive way to start a conversation.

It means you look nice. If someone asks you when the baby is due, that means they think you have gained weight!

I would never think that. More likely it's their way of saying "You're looking well."
