Rome's about 8 times larger than Milan. Milan is more like a small neighborhood of Rome. Rome is also richer, more famous and much more prominent internationally.

Rome, with a population of 3,000,000 is almost three times larger than Milan. As a territory, Rome is about ten times larger than Milan. The Lombardy capital could almost be considered a neighborhood of Rome.
The diameter of the city of Rome (east to west) is about 60 kilometers (from the Ostia neighborhood in the west to the Pantano neighborhood in the east) and it can take up to 3 hours to travel across it.

Rome is bigger which is a scary thought, cause when I went to Milan this past summer, it was hard not to get lost.
