Thomas Selfridge was a passenger on a Wright Flyer when at an altitude of a 100 feet the propeller split sending the plane crashing into the ground. Orville Wright was badly hurt but Mr Selfridge was dead. Thus he became the 1st man to die in flying aircraft accident. Others had died in so called planes but those aircraft never flew.

Otto Lilienthal was the first person to be killed in an aircraft accident (August 10, 1896). He died after a failure of one of his gliders. On his 2,500th flight, a gust of wind broke the wing of his glider, causing him to fall from a height of roughly 56 ft (17 m), fracturing his spine.
He died the next day, with his last words being reported as Opfer müssen gebracht werden! ("sacrifices must be made").

AIRCRAFT covers a lot of territory....as the history channel will attest, many pioneers perished in their flying machine before a person ever flew the first flight...

Probably Otto Lilienthal, he died on 10 August 1896.
On 9 august 1896 he was flying on his glider when wind broke his wing, he fell from 17 m, he died the day after.

Selfridge was the first US Military officer to be killed in an airplane crash. (Orville Wright was the pilot.) There was an Air Force base in Michigan named for him.
