I am not very sure, but if you follow 'singular subject - singular verb' rule, it should be DIES, because the subject here 'one in 27 women' - refers to 'one woman'. If it's like 'two in 27 women' i'd go for DIE.

The correct gramar would be:
"1 in 27 women die of breast cancer." Because you are only talking about 1 of the women out of 27, and (s) would only be added if you' were talking about more than one woman.
