my fiance wants all her wisdom teeth pulled at once. She has two of them that are just barely poking out of the skin but they are coming it at an angle. The top ones are out but just not enough room for all her teeth. Her doc says since she is in her early 20's, the bones are fairly soft and it will not be that bad. He says only 20 minutes and it will be over. Local aneth...Are there any risks to doing it this way? I assume not because the doc says so, but you know how all the horror stories are....

I had all four of mine taken out at once but they put me out completely. I've never heard of it being done with only local anesthesia though, so I don't know about that. Anyhow, I was back at work 2 days later, so it wasn't that bad. I think I was 19 or 20 or so about the time of the procedure.

Having all four 3rd molars removed under local anesthesia at one time is very, very common. The patients are usually a little uncomfortable or nervous and can be premeditated if they are apprehensive about the procedure. The whole mouth will be numb and usually N20 is provided as well to help the patient relax. She needs to remember to breath through her nose through out the procedure if she uses the nitrous otherwise she will loose the effects of it. If the upper teeth are erupted, there should be no problem in elevating them out. The lower coming in at an angle could present a problem where the teeth may need to be sectioned for easier removal. The doctor has seen the x rays and has made his decision as to how to approach the procedure. If he says 20 minutes for the extractions, then they must be simple. I'm sure if he foresaw a problem, he would have referred her to an Oral Surgeon. At her age these extractions shouldn't present any problems since the 3rd molar is the smallest rooted tooth in the mouth, and they are not fused in the bone. What makes most patients sore is that these teeth are so far back in the mouth requiring them to hold open wide for so long. So she will be sore after the extractions and shouldn't do any activity for a few days. After that some jaw stretching exercises will help with the stiffiness. Just have lots of cold juices, iced tea, smoothies, ice cream and soups for her. The first day she should use an ice pack and take the pain medication as directed even if she doesn't think she needs it. It's easier to keep something from hurting before it starts,(especially while the anesthetic is still working) rather than try to stop it from hurting after it starts. The Dentist will probably recommend Aleve or Motrin for inflammation also. They will have more detailed post op instructions for her in written form. She should do fine, just take care of her for a couple of days and be as understanding then as you are right now. Good luck!

Both of my own children were my patients who had the same procedure done, also my daughter had 2 that were impacted.

Do not let a doctor take them out in the chair. I think the only reason that doctor is telling you that is because he is probably not a surgeon and cannot do it with anethesia (spelling???).

I had two taken out at a time (right side, then left side) about a month apart ..... and it was awful. The bottom teeth were "impacted" so they had to drill them into quarters and pull them out in four pieces.

I would get a surgeon to do it if possible. It will be more expensive, but ......if you can possibly afford it.

If you don't know this dentist VERY WELL, then don't let him do this to you. Get a second or third opinion. This doesn't seem like an emergency, so you have time for second or third opinions.

Ask your friends or somebody for a referral to a dentist that has done this before, or go to a surgeon..... that's my advice.

Local anaesthetic is given by weight so unless your fiance weighs forty pounds she's good to go to have all four taken out at once. If she had a general anaesthetic they normally numb the extraction sites anyway - just for comfort after the procedure. You do want them out before they fuse to the bone, or before the roots fully develop and have any chance of damaging the nerve that runs along the root tips when they are coming out so if the dentist says they should come out take his advice. If they are starting to erupt they should come out easier. And all four at once means only one recovery period. It comes down to this - If she trusts the dentist that she goes to then he's not going to tell her to do something that could hurt her. There are potential risks with every procedure, but there's a chance you could slip in the shower and break your nose (my sister's done that twice.) Wish her good luck for me and use it as a chance to baby her.

It's best to get your wisdom teeth out before they attatch themselves to the jaw... or that's what my doctor said when I had mine out... I had an extra one so I had 5 wisdom teeth taken out. The only horror stories my doctor told me were if they were attatched to the jaw, that you had to break them away from the jaw and sometimes that would break the jaw and you would have to have jaw surgery. Another is if the doctor hits a nerve your face could go numb for the rest of your life. But the chances of that are slim. I'd say the earlier to get them out, the better. I was put out durring the surgery and I felt nothing. After your mouth is puffy for a week, and sore, but the doctor gives you a bunch of pain killers.

my bro had all four of his wisdom teeth taken out before they even got the chance to come out since through the x-ray the dentist noticed it wuz coming out at an angle. he went to get the four of them removed at the same time. and according to him...it hurt...a lot...but then again it was because his teeth didnt come out yet and the dentist surgeon had to cut open his gum and like kill the tooth before it got the chance to go out...

The doctor shouldn't pull more than two teeth at once.
