If you ask this question of a good chiropractor the cause is when your bones are not aligned properly. They can be tilted forward, backward or to either side. Not correcting it is the original cause.
Yes feet are different - if they are not too bad than a good arch support can take care of it.

Bone spurs are like callouses. They appear and persists on areas of your body with constant "stress" just like when you have callouses on your hand or corn on your feet.
