Mittlere Reife is the German school level equivalent to O Level/GCSE - ie, around 16 years of age.

Reife on it own does mean maturity and ripeness, but in this context is DEFINITELY nothing to do with sex or maturity or ripeness. "Mittlere Reife" (mittleren in this case) is a phrase meaning that particular stage in the educational process.

"Mittlere Reife" is a specific name of a degree you get when you complete the "middle" form of the German secondary schools. Normally, "Mittlere Reife" is achieved at the age of 16 or 17, so Sylvia was in the hospital for one or two years.

If you translate "Mittlere Reife" literally, you get something like "medium maturity". It has absolutely no sexual undertones.
