yes i think i can tell you, i think what your talking about is a persian rug and there is a fault sewn into the rug, which is i think what you mean by mistake. which is always in the same place for that family who produce that particular rug its a secret fault in the weaving/tapestryof that rug and they do it because they believe only Allah can create perfection not a mortal man. legend has it you could spend a lifetime looking for that fault and never find it.

I remember my RE teacher telling me that & that it was a part of Islam. I ended up having a debate with him on the subject.
The imperfections are traditions & not a part of Islam. There is no Islamic evidence obliging or requesting imperfection to be put on prayer-mats or any other man-made product. (If prayer-mats HAD to have imperfections, then why not mosques, clothes, etc....) In fact Islam does not even obligate the prayer to even use a mat, and in the absence of one, their is no mention of denting/mis-shaping etc... the surface to be prayed on.
It is a tradition that has passed through the generations of prayer-mat weavers, and has become so common that it is mistaken to be part of Islam. It began out of respect for God, that only God can create perfection, so man should not pride themselves in their creations - and so they made intentional mistakes along with the unintentional ones. There is no specific guideline as to what the mistake is - each manufacturer/weaver will design their own mat with their own mistakes. The most common mistake I have found is mats not being vertically mirrored (eg. a small dot on one side doesn't appear on the other), and slight mis-aligning of pictures.

Ah, I think I found it! I didn't know what you were talking about at first because I've never heared of this, but I did a search and found a site that talks about Baloch prayer rugs, and how they are made with a deliberate mistake to remind the owner that only God is perfect.
