Does the Flat fare of $45 (plus tolls) apply when going to JFK or just from JFK .

Also, how much do tolls usually run.

How long would it take to get to the upper west wide to JFK before 5:00 am?

From the upper west side , go UPTOWN to 125th Street , across the Triborough Bridge -$ 4.50 toll - Down Grand Central Pkwy into the Van Wyck and into JFK
Assuming that it is not a Friday or Saturday - NYC bars close at 4:00 AM- you should able to easily make that trip in under 45 minutes.
If you opt for public transport take the subway to Penn station of the LIRR . LIRR to Jamaica Station and then Air Train to JFK
At that hour you don't want to take the subway all the way to Jamaica w/ tourist wriiten all over you. Plus you're only saving 3 bucks and adding about 30 minutes onto the travel time. negative advantage

Yeah, it's pretty much the same going and coming. Last time I checked, though, it was $30 or $35 for the trip. Check with a couple of difference car services. The toll for the Midtown Tunnel or Triboro Bridge is $4.50. You could also take the 59th Street Bridge, though, which is free.

Also, there is now an Air Bus (sort of like a monorail) that connects to the E and F trains, I think. Going that way would be considerably cheaper and take you straight to the airport, but you may not want to deal with the hassle of taking trains and all that.

In any case, before 5:00 am there isn't usually much traffic, so it should only take 20-30 minutes tops.

The flat fare is the same. We've used that method also.
From the Upper west side to JFK should be about 45 minutes to an hour at that time of morning at the most.
I don't remember what the tolls are, but it's less than $10.00.
Experience: Frequent NY traveler.

Yes. (though it would be a lot cheaper to take the subway to Jamaica and catch AirTrain) Generally there are no tolls leaving Manhattan.

A cabbie could probably make it in half an hour before rush hour, but figure 45 minutes.
