Nowhere in scripture does it say someone actually was baptized in the name of the "Father, Son, Holy Ghost". The only scriptural recording of any baptisms after the day of Pentecost (Acts 2:38) were done in the Name of The Lord Jesus Christ. In Matthew 28:19 Jesus told the disciples to go forth baptizing in the NAME (singular) of the Father, Son and Holy Ghost. Meaning there is only one name and that name according to Acts 4:10-12 is Jesus Christ.

It's the Holy Ghost and its found in Matthew 28:19 "Go ye therefore, and teach all nations, baptizing them in the name of the Father, and of the Son, and of the Holy Ghost:" The Holy Spirit is found in the NEW BIBLES.

Matthew 28:19,20
Jesus' last command to his desciples before returning to heaven was to -
"Go therefore and make disciples of all the nations, baptising them in the name of the father, and of the son and of the holy spirit."
OH wait: From Acts to Revelation? Acts 8, beginning in verse 26 talks of Phillip's encounter with an Ethiopian Eunuch. After a discussion, at verse 38 it shows that Philip baptised the Eunoch in a body of water they came across. It doesn't indicate that it was in the name of the Father, Son & Holy Spirit. However, given that he was an early disciiple of Jesus, and given the instructions above that he gave before returning to heaven, you could put two and two together.
