DO NOT PREPARE THIS RETURN YOURSELF! Find a CPA or better yet a an Enrolled Agent of the IRS to prepare it for you. They know all the correct forms and the deductions to look for and criteria that the IRS defines the losses by. You may be contacting the tax professional you chose to help you later on.

My brother-in-law had his house flood from a broken pipe, their return was long and it sent up red flags with the IRS for the $15K in losses they were declaring, they were audited. Since the return was prepared by an EA (the enrolled agent is someone licensed to represent the taxpayer before the IRS) he took all the supporting documentation to the IRS for their scrutiny, but he also found another $4K the IRS owed for the loss.

Your local IRS office will know,you can check the IRS web site.
Most tax software will ask for any such losses as you fill out your tax return and print it in the correct line or form.

i know if you were robbed the police dept give you forms call and ask the fire dept about the forms.
