I laugh whenever I hear someone use the word as an insult. It's so funny cause it's usually directed at a male and douches are for female hygiene. It's like calling someone "a tampon."

Douche is actually French for shower not a soak and it is an Americanism as an insult. No idea how this came about, but to call someone a shower well it is rather pathetic if one does not know the meaning of the word. then again, use of such terms is usually indicative of a lack of vocabulary and basic etiquette.
Back in the sixties there was a well loved British comic called Terry Thomas who used to use the phrase 'You're an absolute shower" This at the time was considered funny as the man himself was very funny.
