The old friend would know me, and would understand why I see to the needs of the elderly person first. This situation is possibly about life and death, and until that is resolved, it's not a time to let personal interests keep you from doing what's right. MY friends would understand me that way. The eye-candy fantasy takes a distant 3rd to the circumstances. She's someone I don't know personality-wise, just lustfully. If she's still around when the important stuff is taken care of, and we're able to hook up; that's points for her because it means she has some character, as well as looks because it would indicate to me that she knew what I was doing was right. Otherwise, had I opted for her and she turned out to be just a pretty face or easy lay, then what I've done to myself and to my friend, and to a person in need is terrible. Definitely, TCB first, then ...whatever happens, happens.

Think back of the nice and sweet time when you were with the 1 you love.. If you don't have 1, just plan for the next perfect holiday. Best of all, to feel glad how much money you have save so far...

Begin to learn yourself. Sit or stand or lie down, be comfortable, in a quiet place with few or no distractions. Or in the middle of a noisy restaurant. But go inside yourself, center yourself in yourself, consult your inner wisdom. Begin relaxing your stomach muscles, your shoulders, your neck. Let your head balance on your neck, adjust your heart rate and breathing. Begin thinking of a calm pool of water in a setting of your choosing. Keep the water calm and when other thoughts intrude, gently push them aside. Shift your focus to something that is present in your life that is important. Let it play around in your head. Just focus on the one idea. You'll find time will pass rapidly this way. But it takes practice. Or, carry around a pocket book and spend your time reading.

I play guitar. But your question is a paradox itself. When you're idle, you're idle. You must attempt to not be idle. If you're doing something to pass the time then you're not idle.
