A round rated by caliber is not always bigger or smaller than one measured in millimeters they are merely different forms of measurement. It is complicated because you are dealing with un-equal units and the millimeter works in whole units and the caliber in decimals of 1.
A .5 caliber round is far larger than a 5 mm round, but a 20 mm round is FAR larger than a .20 caliber round. As a form of measurement 1 caliber is larger than 1 millimeter as you are comparing 1 tenth of an inch to 1 tenth of a centimeter.
A true 1 (1 inch-whole) caliber round would be 25.4 mm.
A .177 caliber (BB) is 4.5mm
A .22 caliber is around 5.6 mm
A 9mm is a .355 caliber
A .50 caliber is 12.7mm
A 12 gauge is .729 caliber / 18.5 mm
It should be noted that caliber measurements are sometimes slightly different (hundreths and thousandths of an inch) in their actual proportions than they are in name.
It is somewhat like measuring engine displacement it can be done in either cubic inches or liters. The equivalent liter measurement will always be numerically smaller than the cubic inches because the liter measures more volume. But the cubic inches will be less than the cubic centimeter measurement (cc). However both these units are working in whole numbers.

This determination of the size of a guns muzzle diameter is not American at all, as it predates the USA by many years and is in fact part of the Empirical systems of measurement.
True, the rest of the world adopted the so-called metric system in the Socialist heydays of the early and mid 20th Century, but many countries and manufacturers that produce firearms still use caliber as a measurement.
Caliber (or Calibre) is simply the measure of the inside diameter of the barrel in inches.
.38 Caliber is 38/100s of an inch.
.50 caliber is 50/100s or half an inch
One inch = 25.4 millimeters

Well a .20 Caliber bullet is smaller than a .22 and a .5mm is the same diameter as the .20 Caliber. Caliber is not necessarily larger than millimeter. The first is American caliberation, the second is standard Continental (overseas) caliberation.
An easy and quick way to convert from mm to American calibration is to multiply by four. Example: A .10mm x 4 becomes a .40 Caliber. Of course, when you are speaking of actual ammunition the .10mm round is more powerful than the .40 S&W. Conversely, if you wish to convert from caliber to mm, divide by four. Your question of .20 caliber vs .5mm (not 50 meter) would be converted by dividing the .20 by four to arrive at .5mm; I hope that helped.

50cal=13mm (approx.)
DJ. .38 refers to the brass diameter, not the barrel diameter. .38 SPL is .357". .44caliber is .429". This is what makes .38 and .44 caliber guns so unique. The measurement of caliber was different in the 19th century and the tradition carried on over to the beginning of the 20th century in regards to those two calibers.

A millimeter is bigger than a caliber.The 22 caliber bullet is under a quarter of an inch and a 22 millimeter bullet is over 3/4's of an inch.

Caliber is the bore size in fractions of an inch, so a twenty caliber rifle would have a bore 1/5 inch (0,20") in diameter. A 50mm cannon would have a bore size of roughly 2 inches.
