My friend is really skinny and trying to get big but he has a family history of heart disease so the food needs to be low in fat or atleast saturated fat. He needs at least 4000 calories a day.

your friend isn't going to increase lean muscle mass substantially on a low fat diet. low fat diets = low testosterone production

the ratio of HDL-C:LDL-C is far more important than total cholesterol values. plenty of fiber and exercise will help to decrease total cholesterol while keeping unsaturated fat intake high (50% of total dietary fat should be unsaturated) will increase the HDL-C:LDL-C ratio

If he wants to gain healthy weight, he should eat a lot of protein and work out (lifting weights and cardio). It doesn't matter how much fat is in the foods...the calories are what matter. Sugar turns to fat if it's not burned off...
However, if he's worried about heart disease, he shouldn't stay away from fat altogether. Some fat is healthy, like the kind you find in salmon (Omega 3).
Tell him to read the labels of whatever he eats (or if there are no labels look up the nutrition facts online). He should stay away from saturated fat and trans fat...no twinkies or fast food!
Bottom line: if he eats a lot of healthy food and works out, he should be able to gain weight.

200 isn't high. But it's pretty straightforward. If you need carbs, just eat some 12 grain bread or something. If you need protein, have some lean poultry, fish, etc. If you need sugars, eat fruit. If you just randomly need food, go for milk, or better yet a bowl of milk & cereal.

You can't go from "skinny" to "big", it doesn't work that way. He'll either end up sick or a fat slob.
