I don't care if she washes her hands every 10 minutes that is still nasty and rude...I would as politely as possible say "Hun...you can have that bread since you decided to breathe all over it".
Regarding the hand washing...she probably gets this from home. Every time she is around I would mention all the diseases people get from not washing their hands and suggest everyone go and wash theirs. When she leaves the bathroom remind her to go back in and wash them. You can set a good example for her.

It's absolutely rude to pull the bread out of the sleeve and stick her nose to the bread. That is completely superfluous. Even if she can't see what the bread looks like through the sleeve, it is not necessary to pull the bread out to smell it, let alone touch it.
As for the hand washing, I don't know what you could do. I guess it depends on how close your relationship is with your friend and her daughter. Perhaps you could make a rule that everyone who gets a snack in your kitchen needs to wash their hands at the kitchen sink first?
And unless you've given the girl permission to make herself at home in the kitchen, she should ask before even thinking about getting a snack, let alone defiling your food and risking your household's health. She's old enough that you could tell her yourself. But again, that depends on you relationship with her and your friend.

Tell her to get her grubby hands off of your bread! And tell her mother that she needs to teach her daughter basic hygiene - in particular 1) don't go grabbing on other people's food and 2) wash your hands after you go to the bathroom! Your friend is raising a little pig!

I would throw out the loaf of bread, spray my bathroom in isopropyl alcohol, cut the 14 year olds hands off and burn down my house. But that's just me.

That is really annoying and she shouldn't have done that. I don't see any tactful way of conveying that you don't appreciate it that she did that, so I would let it go this time. It's only a loaf of bread.

Maybe when dinner is ready to be served, say "alright everyone wash your hands, dinner is ready!"
or maybe jokingly say, "hey, dont stick your nose in that! other people are going to eat it too."
or you could just try talking to your friends and letting them know what is bothering you? but that may be weird.

It depends on past behaviors. If she was otherwise polite i would cut away the area she touched. Perhaps a call to her parents would be in order, but that would be extreme.
