I think it says that God knew what Jeremiah would do before he was even born. He appointed Jeremiah to be a great prophet. I believe that we could extrapolate that to every single human - that God knows us before our birth, and perhaps even our conception. I can't grasp God's limitless power. But I also can't grasp how verses 17 through 19 relate to verses 4 and 5, other than an extension of Jeremiah's purpose, according to God.

A similar thing to what these passages say.
Luke 1:15 For he shall be great in the sight of the Lord, and shall drink neither wine nor strong drink; and he shall be filled with the Holy Ghost, even from his mother's womb.
Psalms 139:13 For you formed my inward parts; you knitted me together in my mother's womb. (14) I praise you, for I am fearfully and wonderfully made. Wonderful are your works; my soul knows it very well.
God considers a person to be a person when they are conceived, not when they are born and have legal standing because men say that they are now a person.

4 The word of the LORD came to me, saying,
5 "Before I formed you in the womb I knew [a] you,
before you were born I set you apart;
I appointed you as a prophet to the nations."
This is saying he knew him before he was thought of, he had a plan for him.

It says that before we were made, God knew that we would be and what we would be. Keeping in mind, God is all knowing does not mean he makes our choices, we have free will. It simply means he knows the choices we will make. God knows every person that has and will be born from beginning to end.
