Legally, none required either for entrance or return to US. Go to CDC website for list of recommended precautions. I predict you are going to love Thailand, and Thai people.

i had the same as above. but depends on where u go in thailand. if you just stay in bangkok and well populated areas full of tourists you will not need what you will be advised to get in the hills.
Check with your surgery, but make sure you tell them where you plan to go.
I traveled throughout the country some years back and my partner at the time had 12 injections

Thailand is safe and you do not need any immunizations. if you are one of those paranoid people - you go to a 'travel clinic' and they will recommend shots to take (Hep A, B, C boosters?...) - they do not hurt to get - but not neccessary for travel.

While no immunizations are required for Thailand or for re-entry into the USA, I recommend that you speak with your personal physician and get any shots he recommends.

be careful while your'e there and you'll be fine.
