As a teenager I never made much either but we(my parents and I) always filed because I usually got back a majority of all taxes paid. Sometimes it was as much as 400 dollars. Use a 1040 EZ form there are only like 10 steps it usually took maybe 15 mins to complete

No you dont have to because it is under the limit of earned income. Good for your kid that he/she is making some money! you might want to think about investing it in an IRA, make your kid a millionare by the time he gets older!!

You don't "have" to but you may "want" to if there were any federal or state income taxes withheld.
