I've already taken Lamisol once and it went away, but now it's back, and even worse. Any advice on getting rid of it?

Those medicines usually only work for a while so your experience of reinfection is common. They have oral doses that work pretty good for a while<
When tissue turns black, then that tissue is probably history. But it will grow back.

Rub VICKS VAPOR RUB in it 2 -3 times a day. It will take a while. Then about 1 -2 times a week after it seems to have cleared up. This is no lie!

Go to the doctor, and go over your foot care. Sounds pretty bad never had it and I stay out of nail salons, because they have it when they don't clean there equipment properly. Clean your shower properly and wash your socks in hot water, find a way to treat your shoes. ASK YOU DOCTOR!

A female friend of mine had the same condition. Just keep taking what you need to until its gone then take it about a week longer and shower.
