If you want them for college purposes then I suggest going to a bookstore (Borders, Barnes&Noble) and going to the reference section. Theyhave tons of books designed to show you how to write the perfect college resume (to include reference letters). If it's for a job then you can get the same type of book. I'm sure there are many examples of templates for references. The books however will help you with wording and it will also help you to understand the types of things that you should and should not say when writing a refernce for someone and when expecting someone to write a refernce for you. The reference letter should be tailored as best as possible for the person and corporation for which it is intended. A lot of times if it's for a school they will have a preferred template they would like used. I would stronly suggest that since they are probably viewing many reference letters and are looking for a certain style. But usually it's just a matter of writing a professional letter with the right words and saying what you need to say in the best way possible.
