Go to your vet and ask for another application and have them scan her so you can write the number on the application.

Go to avidmicrochip.com (it might be avidmicrochips.com) They will have instructions online to get a registration form to register the chip with them. If not, they do have an 800 number you can call.

Another place to register your chip is with the AKC (American Kennel Club) go to akc.org.... look for the CAR (Companion Animal Recovery) link.... follow the instructions.

If you don't know the number on your chip have it scanned at a Vet's office.... HOPEFULLY their scanner will read the AVID chip. If not, call around till you find a Vet whose scanner will read the AVID chip.

HomeAgain has the vets convinced "they" are the only chips on earth! (Not!!!)

did you only loose the paper work for the chip? or all paper work? The breeder may have a copy. Other wise a vet maybe able to help. You may also try www.akc.org

Go to your vet and explain the situation and they will help you. That same thing happened to me twice before. Don't worry.
