Evolution is a scientific fact. It's up to people of faith to decide how to reconcile the two. There are millions of Christians, Muslims, Jews, etc that believe in God and who accept that evolution is real.

No. You can believe in both, or neither.
Actually, even that isn't right. Evolution and religion are two different kinds of ideas. You don't "believe" in evolution. You can accept its reality, or ignore it.
But you can still accept the reality of evolution and create a concept of god that still fits. Very easily.

No. You can accept evolution and still believe in a God or go for many other religions that accept it as well.
Only some religions refuse to accept evolution.
