Peroxide is liquid and comes in a bottle. It is available at any drug store, and most grocery stores.
You'll normally find it in the isle near things like rubbing alcohol. The bottle is normally brown, about 16 oz, and it has directions on it on how to use as a mouth rinse (you dilute it, but I've even used it full strength).
You can also use it to clean scrapes/cuts by pouring it directly on. It bubbles up while cleaning, but doesn't have a burning/stinging feeling like some antiseptics.
For baking soda, just get your toothbrush wet, then dip it into a little container of baking soda ... the soda will stick to the wet brush ... brush away :-)

You can get it at any grocery store or pharmacy. Its in liquid form. Make a paste with the peroxide and the baking soda, add a pinch of salt.
Get a tongue scraper though, the peroxide will foam up, so you will want a tongue scraper to clean things up a bit.

You can either use a toothpaste that already has peroxide and baking soda in it's ingredients (which it will advertise right on the front of the box) or you can get peroxide at the pharmacy, grocery store, or even the dollar store. It's in a brown plastic bottle and will most likely be in the first aid aisle.

You can get hydrogen peroxide at any pharmacy. Dilute it to about 1 tablespoon in a cup of water and use it to gargle and swish around your teeth. Don't swallow it because it is also used to make you throw up if necessary...just like epicac syrup.
