You must fill out an application at the consulate, and bring your passport plus two other photo id's,

You cannot apply for a visa through a travel agency any more. In India, you can apply in Delhi, Mumbai, Calcutta, or Chennai. Since you haven't said where you live, here are all three web pages with complete instructions:

You can't. We have gotten very paranoid so you can't. But when you apply, you should have proof that you have reasons to return to your country - wealth, family, job, relatives. Otherwise, forget it.
