My cat does the same thing. She's done it since she was a kitten and she's about 4 now. The solution I've found is buying one those litter boxes with a lid and an opening that the cat can get through (TidyCat makes a really good one!). They're fairly inexpensive and can be found just about anyplace that sells litter boxes. Sometimes my cat still misses the box but not as often as she once did. Hopefully this will work for you as well!

A few people have suggested an enclosed tray, which is a great idea if your cat just has a poor aim. If this is something that has only recently started to happen, I'd recommend taking it to the vet to rule out a kidney infection or cystitis which mean your cat just can't get to the tray quick enough. Make sure you keep your litter tray as clear of wastle as possible - no cat likes a dirty tray.

Some cats will avoid hooded liter boxes, especially if it's a change. My Mom's cat does the same thing and would not use the hooded box. She found a liter box with deeper sides and it solves most but not all of the issue. I know she had trouble finding a very deep one. She still takes the precautions others have suggested just in case. i.e. newspaper or something to catch the misses.

I have an older cat that does the same thing (I have six cats). My solution was to buy one of those long plastic slide under the bed storage boxes to use as a litter box. No more accidents unless I do not clean it out twice a day.

Bigger tay and use something like butcher paper or newspaper for under like a placemat. Just ball and trash it when it's foul.

place puppy peepee pads on the floor around the outside of the box, so when she misses it will be on the pad.
