The flayed skin is held by St. Bartholomew as a representation of his martyrdom and the skin itself is a self-portrait of Michelangelo himself.

A second self-portrait appears in the lower left corner in the figure of the man looking up to the centre of the painting.

Link goes to a detail of St. Bartholomew and the flayed skin.
