Use a nice poem for the church prayer. Your ideas are good for the church people to hear. I want to share how I relearned to pray in a way that changed my life. I began to ask God instead of telling Him what should happen. It was then my learning increased and I became luckier in life . All I had to do was ask a question and wait for a 'yes' when God agreed or a 'no' when God said the idea was wrong.

Ya see, when we're born, we're born with a live body and a dead spirit. This is what died that day Adam sinned (if you note, the next chapter, they need a sacrifice to even approach God.) Ever since, all humans have dead spirits. This is what is "born again" (John 3:3) when we accept Jesus as our Savior. For, as Jesus clarified in the next chapter, "God is a Spirit: and they that worship him must worship him in spirit and in truth" (John 4:24). Thus, without a living spirit, one cannot pray. However, once your spirit is reborn, you also receive the Holy Spirit (Luke 11:13). HE will teach you what to pray. And without His help and guidance, you really are wasting your time. And theirs.

Look up prayers on the internet and pick the one that seems best suited to your situation. Memorize the prayer, and recite it. If you want something unique, look for an Orthodox prayer book - they are all over the internet, and they won't contain anything that is in opposition to the beliefs of your parish.

Make a quick outline on a 5x7 card, and make it from the heart.
There is no formula for prayer, just talk to God like He was in the sanctuary, which He is, and address Him as Father.
Here is a sample:
Father, we ask your blessings on every member of this church, on their relationships, their extended families, their health, and their finances.
We ask your blessings on the community that surrounds this church, that they may know you as we know you.
etc.etc.etc.
Got it?
The only purpose for the card is so you won't forget something on your list.
Also make it brief.

It depends on what denomination you are. Are you talking about writing the prayers of the faithful for Catholic mass? In which case you can write stuff down. The prayer itself isn't determined on whether you feel sincere or not but on God who hears and our prayers even when we are not 100% sincere, yet still works according to his will.

Let your attention be focused on what you are doing, realizing that it is to God, the Almighty, the Creator and Judge of all men to whom you are coming. Don't be distracted by people. Don't rely on formulas and ritual. If it was you someone else was praying for, how would you want them to pray?
Wouldn't you want them to be truly concerned for your specific need?
Wouldn't you want them to really expect that God would listen, and answer your prayer?
Wouldn't you want them to pray with compassion? with thoughtfulness?
Above all, remember that we can approach God on one basis only - because Jesus died for us and rose again from the dead, removing every obstacle to our approach before a Holy God. Come then praying in the name of Jesus, trusting in the goodness of God as shown through what He did for you at Calvary, and believe that He will listen, for Jesus sake, even to you.
