Hi Chino:

You probably mean that you omitted a W-2 form, not the W-4. They are two different forms, and only the W-2 gets filed with your income tax return.

The easy answer is that you file an amended return. There are plenty of instructions for filing the amended return, and the forms can be downloaded from the web.

You'll have to file an amended return on form 1040X. File it before April 16 so you don't get subject to interest and possible penalties.

if you are getting a refund you should wait until you receive it. then file an amended return. even if you had to pay in, wait until you know they have processed the return. if you send them a corrected form now it will screw up their system and take longer to process the forms. i know because i had to do that last year with both of my kids.

amend it and if you owe more do not cash the refund check they send you ( if you are getting a refund) they will charge you interest and penality but if you do not cash the first check they mostly waive the interest and penalty
If you paid in on the first one and owe more just send the difference

you file a 1040X and if you owe money, you pay it with the amendment if you possibly are due more refund, it will be sent to you eventually, there is no direct deposit on a 1040X
