The first and most obvious is bad past experience or lack of experience. Either of these can be cured by good experiences.
The rest depends upon the reason or rational for having sex or making love. If the goal is just physical release for both parties, then she needs to pay as much attention to her partner as she does to herself. Experimentation with a close partner may be the key. Being totally passive, without sound or motion may get some partners excited, but these may not be considered normal.
Leaving the bed or (worse) staying in bed and answering the phone or watching television is a No-No. Starting conversations relating to topics of work or other partners is a complete turn off.
A steady routine or over pickiness will usually place her in the "not so hot" category, not into horrible. Suspect anything that reeks of "I (or we) always do this.."
If the bedroom activity is to express your deepest felt emotions to a partner and not just for physical release, then the only thing that would make a woman horrible is feeling nothing or trying to hide negative feelings. If she properly and honestly expresses her feelings, she may be considered inexperienced or weird, but never horrible. As long as she is open and honest, she will do alright. This includes statements like "I have no idea of what I am doing or what to do to express what I feel."

There are a multitude of factors here. There is someone for everyone, one persons horrible is another persons wild. I find men are most excited when I am less concerned with them and more self centered in the bedroom. However, I haven't met a man who doesn't require you to go down on him now and again to keep him happy. Play with your breasts, stimulate your clitoris, know how to satisfy yourself, so you can teach him, ask for the position you want, do what makes you happy and men are generally happy too. Confidence is synonymous with sexy....buy books, buy sex-education tapes, whatever you need to get yourself educated on what you like, then find a guy who gets off on the same stuff. Worry more about you than him, if he thinks you're horrible...well, you may be, however, you may just be wrong for eachother in that area.

Why have mostly women answered this question? Are they experienced with other women? I think it's because to men, there is no such thing as a bad lay

Not into it. No movement. No life. I think if, afterward, a guy could think to himself "Wow, well I could have done that myself..." she is bad in bed.

LOL ( I agree with the answer above)
My guess would be:
Being absolutely silent
Laying there like a log (like everyone else said)
Not being clean
Not knowing how to please
