My family has did it for years, just make sure you cut them into 3" lengths and put them in when you first get the sauce boiling. That way they get boiled through, before the sauce is set to simmer. You'll love the flavor it brings to the sauce if you use a good quality sausage. Try boiling a pepper, such as a New mexico (fresh not dried) for even more flavor. My family also boils chicken in the sauce in the same way. If you've never had it this way try it, boneless skinless works best. My non Italian friends love this and always request it when having dinner at my house.
Also as far as the grease in the sauce comment, you need only spoon it out as it gathers on top.

I would Cook, bake, broil or pan fry the Italian sausage. It gives a nice color and taste. It may work well by simmer raw. Dunno..never did that.

yes of course.... you do need to cut the product into smaller pieces for even cooking, but by simmer, I assume a low heat for a long time, like a crock pot? As long as you have cooked the sausage thoroughly, then you will not be at risk for foodborne illness.

it is best to cook it first as it will take to long to cook in the sauce and by the time it would get cooked you will have no sauce left.

it's safe, but I would cook it first to get all the grease off it. I wouldn't want all the grease in my sauce.

i would recommend broiling them first. takes about 10 minutes. then put them in your sauce.
