Normally, BPH or an enlarged prostrate problems are common in aged men. In your case, most likely, it's just a question of anxiety. I, myself avoid using public toilets as much as possible,
However you can overcome this anxiety by going to upscale restaurants who have better facilities rather than gasoline stations. A lot of these fast-food restaurants do keep their facilities pretty decent due to health codes impose by the diligence of the Dept of Public health.

Thanks for the additional details. Now, this appears to be of a major concern. It's not just a simple case of GAD (Generalized Anxiety Disorder) esp when you hold your urine x12 hrs. and the length of time it takes you to void. Hematuria ( blood in the urine) or dysuria (diffficulty ) or excruciating pain upon micturation (urination).and fever, at times, are symptomatic of UTI (Urinary Tact Infections) or suspected bladder or kidneys or urethral stones. UTI also exhibit with severe back pains, and fever. Believe me, the pain is so unbearable. in both cases. I know because I have had two episodes of UTI's.
I encourage you to consult a urologist immediately.; to allay your fears. He might want to do a cystoscopy and check for stones, or malignancy Most Urologists will try to treat an unstable bladder by behavior modification first, then medications. This includes the following: Void frequently on a time schedule before you get the urge. Void completely even if it takes some time to empty. Drink fluids in moderation. Avoid caffeine products.
ISuspected causes for your condition to be considered :might be UTI( Urinary tract infection);Urinary urgency or unstable bladder.and /or urinary retention.

Other systemic and nervous system conditions that affect bladder emptying deleteriously are: diabetes, back injury with a “ pinched nerve”, stroke, multiple sclerosis or benign prostatic hypertrophy. (BPH) . BPH exhibits requency, urgency , in small amts ; and intermittent flow. and urinary retention.
Urinary Retention( ischuria ) is a lack of ability to urinate. It is a common complication of benign prostatic hypertrophy (also known as benign prostatic hyperplasia or BPH), although anticholinergics may also play a role, and requires a catheter. Various medications (e.g. some antidepressants) and recreational use of amphetamines are notorious for this.
Signs and Symptoms
Urinary retention is characterized by poor urinary stream with intermittance, straining, a sense of incomplete voiding and urgency. As the bladder remains full, it may lead to incontinence, nocturia (need to urinate at night) and high frequency. Retention is a medical emergency, as the bladder may "distend" (stretch) to enormous sizes and posibly tear if not dealt with quickly. You should go straight to your Emergency Department as soon as possible if you are unable to urinate and you have an extremly full bladder. The water can also pass back up the Ureters and get into the Kidneys, causing damage or faliure to the Kidneys.
In the longer term, obstruction of the urinary tract may cause:
Bladder stones
Loss of detrusor muscle tone (atonic bladder is an extreme form)
Hydronephrosis (congestion of the kidneys)
Hypertrophy of detrusor muscle
Diverticula in the bladder wall (leads to stones and infection)
Causes
Benign prostatic hypertrophy
Prostate cancer and other pelvic malignancies
Congenital urtheral valve abnormalities
Detrusor muscle dyssynergia
Circumcision
Damage to the Bladder
How you can help yourself if you are in Urinary Retention
Run a tap when trying to urinate
Try a hot bath, it may help you urinate, it may help if you are in water and may give you the urge to go
Use a intermittant catheter if you have one ready to hand, to drain the bladder of the fluid in it.
Reduce your fluid input if you are in retention, to avoid further distention of the bladder.

Having a "shy kidney" or "shy bladder" is not unusual; many young men have problems urinating in public bathrooms. If you have problems urinating in private, that's when you need to worry about an enlarged prostate or other problem. If you are concerned, talk to a doctor, but if that is the only symptom, stop worrying!

Mikey, any time you have difficulty starting or controlling your urine flow, prostate enlargement could be suspected and it's good that you knew to think of that, HOWEVER d/t this problem ONLY occuring in public surroundings, this would not be an initial concern.

If it's only in public places, it seems more like an insecurity. You probably just don't like the idea of showing yourself in a semi-public place. Go in the stall to pee.

Possibly. You may also have what is referred to as "bashful bladder" syndrome. To be certain, you should be examined by a physician.
