take some kind of wire, hanger would be the best.
make the end part into a hook and just scoop it out.
be carefull it might scratch you

That pen cap is digging into your skin-you need to go back to the one who applied your cast and tell him the problem. That pen cap will cut through your skin and may get infected. This happened to a friend's son-the boy didn't tell his father and when the cast was removed, there was the pen cap and a big wound with foul smell on his arm. Took about a month to heal but he had a big scar.

you need to go get it recasted. not only to get the pen cap out but also to make sure you didn't break the skin which would be really bad. hate to say it but that's why they tell you not to stick anything in your cast. if it starts itching again get a hairdryer turn it on low and put it up againist the cast, the vibrations will help get rid of the itch. if that doesn't work take some benedryl

Push it out with a knitting needle. If you cant do it then you need to 'fess up and get the cast removed and replaced. If you don't you could end up with a nasty sore from the pressure of the pen cap;

stick another small thing down it but something that can't get stuck. If that doesn't work, go back to the doctor and tell them. They might be mad but it's better. If it's left, it might make it heal wrong and make it worse. SO yeah. I did the same thing with a ruler and my leg cast, the ruler broke and i coudn't get it out. I had to go in. Heh. I hope it goes okay and you can get it out on your own. I know, they ITCH like **** lol.

If you can't grab it with a pair of long nose pliers, then cut a hole ,over the cap, and yank it out.

PULL IT OUT WITH THE HOOK END OF THE HANGER & DON'T PULL THAT STUNT AGAIN!!!!!!!!!!! OR GO SEE THE DOCTOR, MAYBE CAN JUST CUT OUT OR OFF THAT SECTION.
