It depends on the state first, then insurance company.
If your state fraud laws require the insurance company do a physical inspection of the car then yes.....otherwise, it's up to the insurance company. For example, if you're in CA then an inspection is mandatory, it's not in NV.
Inspections are done 1) to prove the car exists and 2) to check for prior damage so you can't claim it on your policy. Both of these are HUGE areas for insurance fraud.

It depends on the company and where you live. I've had to send in pics before, as well as another company requesting an "inspection" by one of their certified repair centers. My current company, (Nationwide) just asked if there was any body damage or rust on the car, no pics or inspection required. They just want to make sure that you're not getting full coverage so that you can file a claim for damage that's already been done to the car.

Some insurance companies do require a physical inspection of the vehicle to verify prior damage or to document any custom equipment. Others will take your word for things. I've had it go both ways.

Yes, most companies inspect the vehicle before they will write a full coverage policy. You may want to try getting a quote online. I am paying less than ½ of what I was before I did.

depends on your insurance company. I recently bought a new car and when I needed insurance all I had to do was call and tell them the VIN number and stuff, and boom, I had insurance. This is State Farm by the way. They've never asked to inspect. Esurance was also the same way..just give them the info and that's it.

Depends on the insurance company. So just take the info from the title for its condition and miles, some want to actually inspect the vehicle inside and out and then base their insurance on that.
I've had it both ways.

As a general rule, no. But it would depend on the insurance company and their rules. With older cars they will sometimes request to see it so they can note prior damage. With a new car they generally don't ask to see it.
