My sister-in-law takes my nephew's favorite loud toy and lets it keep him occupied.

There is a product (cream) called "Emla" made by Astra. It "freezes" the area where the needle will go. It's rather expensive but you can check it out on the web at www.emla-usa.com The disadvantage is that you need to know where the needle will be inserted about 20 minutes prior to the procedure.
A lot of his reaction will depend on yours. If you are calm and reassuring, he'll likely be fine. Just remember that he will have to have this procedure done again at some point in his life...so teach him now that sometimes, although unpleasant and may be a little hurtful, it will be over quickly and is for his own good. For example, my daughter is terrified of bee stings. Not that she's never been stung, but because my spouse is afraid of them. He's not allergic to them, just afraid of them and has passed it on to her. I don't like picking up earthworms but picked one up for her when she was small. She has no fear when it comes to baiting her own fishing hook! So bite the bullet and explain in at 4 year old's level what is going to happen and reassure him it will be over quickly. What ever you do, don't tell him it won't hurt because it will but just for a minute. Save the "it's for your own good" for when he's old enough to understand. Sure he can sit on your lap and if your calm, he will be too.
Good luck!

I really don't know, it took four nurses to hold my daugther down to take her blood at eight years old. I could still hear my girl talking smart, the nurses made me wait in the waiting room.

Maybe the nurses should take him in and you can wait in the waiting room. If you react then he will also react.

Tell him that you will give him some shopping money or for a treat after he sees the nurses to take a blood test. Explain he will be getting his blood taken.

Yes, he can sit on your lap, and restraint will be harder on you than him, dear one. Be strong and do as the nurse tells you. The nurse most likely has much experience.
