There used to be an underground passage, from 7th Ave to 6th Ave/Greeley Square back in the 1970's, not any more. (You can see the boarded up area in the passagway between the 34th Street B/D/F/N/Q/R/V/W area and the PATH/Manhattan Mall, the first staircase coming down from Payless on 34th Street, make a right).
Maybe you can't avoid going outside but you can minimize it. Walk to the main ticketing area in the LIRR concourse, then use the 7th Ave exit behind you (it's between the ticket area and McDonalds.) Diagonally across you is Macy's, enter store to 6th Ave side. At the other end, go to the right, then outside to the corner of Broadway and 34th Street. Enter subway there but do not use turnstile. Instead, go to the other side of the booth and you will see the PATH passageway. Follow signs to PATH.

No, there isn't. The only thing you can do is go through the MidManhattan mall - use the back entrance, on 32nd or 33rd Streets. Then use the escalators to go downstairs. You can get the PATH by going to the lowest levels. However, when the mall is closed this way of entering is closed too.

I think you can take the A,C,E to 14th St. and take the L one stop to 6th Ave. You can get on the PATH at that station.
*actually, I think you can take the A,C,E right down to West 4 St. and be able to get the PATH there. It's easier than transferring to the L. I don't use that station ever so you'd have to double check that.

No, there isn't. Penn Station runs along 7th/8th Avenues, and as far as I know there is not a connection over to the 6th Avenue subway lines.
The closest you can probably get is the exit for the 1 trains that is over between 7th & 6th. It lets you out closer to 7th, but not too far from 6th.

You can take the A/C/E to the world trade center, then take the path there to newark, and then the path there to 33rd (after transferring in JC).
Maybe if you give you "end destination", we can give better answers
