In my experience, it's highly likely if your flight gets re-scheduled. Having connecting flights increases the likelihood. A direct flight with no changes in schedule or carrier is the safest.

Carry as much as you can with you. Make sure you have emergency/backup/survival stuff with you (minimal toiletries, change of clothing, valuables).

Follow published advice for avoiding it (see links).

If you have to deal with claims/baggage people, be nice, but be as firm as the need dictates. For instance, if you've got lost luggage with needed meds in it, that would be an incentive for them to help you out. I've found that having a good reason like that and being polite and patient yield the best results. The people that might try to help you are not the ones that lost the luggage.

Normally, everything works out fine.

Your chances of having lost, delayed or stolen luggage depends on the individual airline. From the attached link on the latest statistics, your odds go from 3 per 1000 up to 16 per 1000 passengers that you will have a luggage problem. However, in almost all cases, the luggage does eventually end up with you. Plan for the worst, carry an extra days clothes as carry on should your luggage be delayed.

Out of maybe 100 flights in my life, I only had one missing bag and I got it back a week later(United).
