All you need is a picture ID, such as a student ID, driver's license and your birth certificate. You need to go to the Post Office or a government building to get the passport, also it will cost about $100.00 you get it in 6-8 weeksm but if you need it in 2-3 days figure close to $200.00

Go to your local post office. They have a person, and maybe a special line where there is a person to help you fill out the papers and take you pic. you need to bring a notarized copy of your birth certificate, social security card and marrriage license if your name is different. They will send it all to a passport office, and you'll get it in about 2 weeks in the mail. Hope this helps.
