My sister is constantly twitching her hands and she always has to be holding something in them. she loves to fold up paper in small pieces and sometimes she feels she has to tear it up. If there is nothing around for her to hold she will rub her knee or something, what could this be?

It could be a Motor Tic Disorder. Perhaps even anxiety (although what you are describing is not really consistent with an anxiety disorder). Sometimes these disorders are confused with something more serious like Huntington's Chorea - again, it doesn't really sound as though this is the problem either.
Perhaps she's just active and jittery - alot of people are like that. It's just hard to say without know her age, how long this has been going on, her mental status, etc.
I would suggest a visit to the doctor if you are really concerned.

How old is your sister? When did this behavior start? Does she have difficulty making eye contact or mirroring emotions with others?
[I ask these, because it is possible that she may be on the autistic spectrum.]

Were there any significant events in her life that might have caused great stress?
[Possibly some sort of anxiety, obsessive compulsive disorder, etc. Something to think about.]

called o.c.d obsessive compulsive disorder she needs meds for this. get her to go to the Dr or it will get worse.

ocd maybe sounds like intruding obsessive thought and i'm sure if she doesn't act on those thoughts she gets extreme anxiety.
